Concerning "Complementary and Alternative Health Care Practices"

The State of Minnesota has not adopted any educational and training standards for unlicensed complementary and alternative health care   practitioners.

  This statement of credentials is for information purposes only.

1. Reiki By Kim is located in Minneapolis, MNand may be reached at 612-220-3909.

2. Kim Antonson, Reiki Practitioner, has worked with Reiki energy therapy since 2004.

3. Kim Antonson, Reiki Practioner's, approach is to work as an active partner with you and offer energy healing to aid you in your healing process.

4. Kim Antonson, Reiki Practitioner, will seek consultation and supervision with colleagues as needed and other health care professionals as is appropriate to the case. As required by Minnesota law, Kim Antonson, Reiki Practitioner, will not provide medical diagnosis or recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis from a licensed medical or mental health practitioner, they may seek such services at any time.

5. Clients of Kim Antonson, Reiki Practitioner, have a right to file a complaint with the office of Complementary and Alternative Health Care Practice at 85 East Seventh Place, St. Paul MN 55101. Phone: (651) 282-5623.

6. The current fee for a Reiki session with Kim Antonson, Reiki Practitioner, is $60 for initial appointment, $160 for a series of three sessions. Payment is in cash or check at the time of the session. No insurance or other institutional second party payments are accepted. The client has a right to reasonable notice of changes in fees or service. If a client does not show up at appointed time or cancels an appointment less than 24 hours of the scheduled appointment, Kim Antonson, Reiki Practitioner, reserves the right to charge the agreed upon fee for the missed visit.

7. The client has the right to fair courteous treatment and to be free from any verbal, physical or sexual abuse by Kim Antonson, Reiki Practitioner. The client has the right to discontinue treatment at any time. Further treatments are with the mutual consent of the client and Kim Antonson, Reiki Practitioner.

8. Records of sessions are confidential as is any other information arising out of the course of a therapeutic session, unless release of the records are authorized by you in writing or otherwise required by law.

9. The client has the right to access any records kept and has the right to request the sharing of any information with the client's other care providers in accordance to Section 144.335 of Minnesota Statutes.

10. Prior to any provision of service, the client must sign a written Consent for Treatment attesting that the client has received this Client Bill of Rights.
